Student Health Requirements

St. Thomas More Cathedral School uses Magnus Health, our secure electronic health record system, to manage all student health information. Magnus can be accessed through the icon bar at the top of our website.

 

Important Dates

  • Magnus forms available: June 16, 2026
  • Submission deadline: July 15, 2026

 

Required for All Students attending STM:

Each year, all families must complete or renew the following in Magnus Health:

  • Conditional Questions
  • 24-Hour Emergency Permission
  • Permission to Share Information

 

Additional Requirements:

  • New Students, Rising Kindergarten, and Preschool Students (including returning preschoolers) – Must submit a completed Virginia School Entrance Health Form signed by your child's healthcare provider.
  • Rising Seventh Grade Students - Receive the required Tdap and Meningococcal Conjugate (MenACWY) vaccinations.
  • Upload proof of immunization to Magnus by July 15, 2026.

 

Students with Medical Conditions:

If your child requires medication during the school day or has a chronic medical condition, additional forms completed by your healthcare provider are required each school year. While forms are available on our website, all completed documentation must be uploaded through Magnus Health.

 

Common Additional Requirement Forms include:

  • Asthma: Inhaler Authorization Form and Asthma/Allergy Action Plan.
  • Attention Disorders (ADHD/ADD): Medication Authorization Form for medications administered during the school day. As controlled substances, we strongly prefer families partner with their care providers to arrange doses outside of the school day.
  • Type 1 Diabetes: Diabetes Mellitus Management Plan, a Medication Authorization Form for insulin, a Medication Authorization Form for Baqsami/glucagon, and a Diabetes Quick Reference Guide.
  • Seizure Disorders: Seizure Action Plan and Medication Authorization Form for any maintenance or emergency medications.
  • Food Allergies: Food Allergy & Anaphylaxis Plan and medication authorization forms for emergency medications.
  • Other Chronic Conditions: Please contact the School Nurse to determine the required documentation and medication authorization forms.

 

Whenever possible, medications should be administered at home. If medication must be given during the school day, appropriate authorization from your child's healthcare provider is required.  For any additional questions regarding health forms or medical requirements, please contact the School Nurse at [email protected]
