Required Forms

Parents must complete the following required forms for each of their enrolled children prior to first day of school. Students will not be allowed to attend school until forms have been completed and turned into the school. 

 
These include: 
  • Handbook Agreement
  • Media Release
  • Dismissal & Pickup Authorization 
  • Health Forms uploaded to Magnus  
This process will walk you through all of the required forms to ensure that you have met the requirements. Documents and updates to Magnus will be reviewed and processed by our office staff.
 
Deadline: August 15, 2025
 
Please note that systems such as Magnus take time to upload and record all information. The deadline above gives the office staff adequate time to process your required forms. If you miss this deadline, your forms may not be processed by Sneak-a-Peek on August 20 and you will need to meet with office staff before proceeding to your child's classroom.
