All student health requirements and forms must be completed through Magnus Health. The July 15, 2026, submission deadline has passed.
Required for All Students: Families must complete or renew the following in Magnus each year:
- Conditional Questions
- 24-Hour Emergency Permission
- Permission to Share Information
Additional Requirements:
- New students, rising kindergarten, and preschool students (including returning preschoolers) must submit a completed Virginia School Entrance Health Form signed by a healthcare provider.
- Rising seventh graders must have required Tdap and Meningococcal (MenACWY) immunizations and upload proof to Magnus.
- Students with medical conditions, including Type 1 diabetes, seizure disorders, asthma, or food allergies requiring an EpiPen, must have all required 2026–27 healthcare and medication forms uploaded to Magnus.
- Students requiring medication during the school day must have the appropriate medication authorization form uploaded to Magnus before bringing medication to the school clinic.
All Magnus requirements must be completed before students may attend school.
