Back-to-School & Other Forms

In preparation for each new school year, St. Thomas More Cathedral School asks parents to complete a series of forms which are posted here for your convenience.

Please note: In September, the K-8 book fee of $250 will be billed in FACTS in addition to half of the $100 PTO fee. The PTO dues are split into 2 payments of $50 with the second payment charged in February.

Upon electronic submission, you will receive an email receipt confirming your order.

Powerschool Account

All new families are required to set-up an online Powerschool Account. A letter will be mailed out on August 8, 2019 with your login and password. 

Returning families MUST log in to their existing accounts to confirm and/or update demographics annually, including address, phone, permission for emergency care and emergency contact information. 

Parents receive the weekly school electronic newsletter based on the contact information in Powerschool. Please contact stmoffice@stmschool.org if you need login assistance.

Dismissal & Pick-Up Authorization*

Please click on the link at the left and complete the online form to provide dismissal & pick-up instructions for your students. All families must complete this online form.

PreK-8 School Supply Lists 

Parents may access classroom supply lists by clicking on the link to the left. Supplies may be purchased by you separately or by ordering a school supply kit in June. Questions should be directed to dad@madisongerety.com

Register as a Volunteer*

Required for all new families and all returning families who have not yet registered. Please fill out the information completely, including secondary contact/spouse details, privacy settings, interests, and child’s name. *Please note that all volunteers must complete Diocesan Child Protection training and submit all VIRTUS paperwork before becoming a volunteer.

 

Diocesan Child Protection/VIRTUS Paperwork

All STM volunteers must complete Diocesan VIRTUS training, background check, and other required paperwork before volunteering at STM . Please click on the link to the left to access the necessary paperwork. Completed hard copies should be submitted to Mrs. Neubauer in the school office, preferably by the start of the academic year. Grades K – 5 Technology Use Agreement* 

All K through 5th Grade students. Please have your K-5 student sign the document linked at the left governing computer use and internet access at STM. If you have more than one student, please submit an individual agreement for each student. 

Grades 6 – 8 Technology Use Agreement*

All 6th through 8th Grade students. Please have your 6th, 7th and/or 8th Grade student(s) sign the document linked at the left governing computer use and internet access at STM. If you have more than one student, please submit an individual agreement for each student.

Pre-K Handbook Agreement Form*

All Pre-K students. If you have more than one student, please submit an individual agreement for each student.

K-8 Handbook Agreement Form*

All students. Parents of K-8 students must sign and have your student sign the handbook agreement and submit the form. If you have more than one student, please submit an individual agreement for each student.

Confidential Health History Update

All new and returning families must complete this form for each student. Fillable form should be completed, printed, signed and returned to the school nurse by the above deadline. Questions? Email nurse@stmschool.org for assistance.

School Entrance Health Form / Immunization Record

All Pre-K, Kindergarten and new families must download this form, have it completed by a healthcare professional (MD/NP/PA), and return it to the school nurse by the deadline listed above

 

Permission for Emergency Care

Form required by the Diocese of Arlington for all students.

Medication Authorization Form

Please complete and deliver with either over-the-counter or prescription medication to the clinic as needed. Prescription medication requires MD/NP/PA signature. Forms and medication sent in with students will not be accepted – a parent or guardian must accompany this form.

Severe Allergy Action Plan

Download and complete for students with severe food/insect allergies- MD/NP/PA signature required and MUST accompany Epi-pen and Antihistamine forms below.

Asthma Action Plan

Download and complete only if your student requires an inhaler. Must accompany inhaler form below.

Inhaler Administration Form

Download and complete only if your student requires an inhaler. Must accompany Asthma Action Plan form above. 

Antihistamine Form

Download and complete if applicable. 

Epi-Pen Form

Download and complete if applicable. 

 

Media Release for Minors*

 Required by the Diocese of Arlington.

Field Trip Permission Form

Complete as required by classroom teacher. 

After-Hours Facilities Use Form

CYO coaches, Scout leaders, and as applicable –

The signed consent form must be returned to the STM office no later than one week before you would like to use the facility. Please note that Religious Education and Youth Ministry requests will take priority for classroom space and during the basketball season, CYO Basketball requests will take priority for gym space.

You also agree that no certification authority or other third party verification is necessary to validate your E-Signature, and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting agreement between you and St. Thomas More Cathedral School. You are also confirming that you are authorized to enter into this Agreement.
