Back-to-School & Other Forms

In preparation for each new school year, St. Thomas More Cathedral School asks parents to complete a series of forms which are posted here for your convenience. Most forms can be accessed by clicking directly on the form name. Please read the instructions to the right of the form name to assess whether a particular form or online submission is required for your family/student. All hard-copy forms are due in the office by July 19, 2017 and all online submissions are due on the same date unless otherwise indicated in the instructions.

MANDATORY ONLINE FORMS & LISTS


Incidentals Order Form
Required online submission for all new and returning families. Use this form to place orders for books, milk, extended day registration, September hot lunch, pizza and Chick-fil-A lunches. Upon electronic submission, you will receive an email receipt confirming your order.
Powerschool Account Set-Up

All new families are required to set-up an online Powerschool Account . Returning families are asked to check their existing accounts to add sibling data (if applicable) and ensure that all other information is current.


Your Powerschool account allows you to fill out your Permission for Emergency Care forms and Waiver Right to Object directives online. It also collects information for use in the school directory. Parents will be able to choose their preference about how much information is shared in the school directory, which PTO will make available to all families as a PDF document.
 All families, new and returning, must complete or update their Permission for Emergency Care forms and Waiver Right to Object directives in Powerschool by August 15 (the soonest you can access the portal is August 1). Dismissal & Pick-Up Authorization Please click on the link at the left and complete the online form to provide dismissal & pick-up instructions for your students. All families must complete this online form.


PreK-8 School Supply Lists  Parents may access classroom supply lists by clicking on the link to the left. Supplies may be purchased by you separately or by ordering a school supply kit through SchoolKidz. To access the STM school supply kit web portal, click here and use School Account #28057. To arrive in time for Thrilling Thursday, all SchoolKidz orders must be placed by June 23, 2017. Pre-K parents only: please note that there is no SchoolKidz kit for Pre-K; please visit our Supply List page for all Pre-K information.


Register as a Volunteer* Required for all new families and all returning families who have not yet registered. Please fill out the information completely, including secondary contact/spouse details, privacy settings, interests, and child’s name. *Please note that all volunteers must complete Diocesan Child Protection training and submit all VIRTUS paperwork before becoming a volunteer.



MANDATORY HARD COPY FORMS


Diocesan Child Protection/VIRTUS Paperwork All STM volunteers must complete Diocesan VIRTUS training, background check, and other required paperwork before volunteering at STM . Please click on the link to the left to access the necessary paperwork. Completed hard copies should be submitted to Mrs. Neubauer in the school office, preferably by the start of the academic year. Grades K – 5 Technology Use Agreement All K through 5th Grade students. Please print, sign, and have your K-5 student sign the document linked at the left governing computer use and internet access at STM. If you have more than one student, please submit an individual agreement for each student. 


Grades 6 – 8 Technology Use Agreement All 6th through 8th Grade studentsPlease print, sign, and have your 6th, 7th and/or 8th Grade student(s) sign the document linked at the left governing computer use and internet access at STM. If you have more than one student, please submit an individual agreement for each student.


Pre-K Handbook Agreement Form All Pre-K students Parents of Pre-K students must download, print, sign, and have your child sign the Pre-K handbook agreement and return the form to Pre-School Director, Sarah Conrad. If you have more than one student, please submit an individual agreement for each student.


K-8 Handbook Agreement Form All students. Parents of K-8 students must download, print, sign and have your student sign the handbook agreement and return the form to the school office. If you have more than one student, please submit an individual agreement for each student.


Confidential Health History Update

All new and returning families must complete this form for each student . Fillable form should be completed, printed, signed and a returned to the school nurse. Questions? Contact Nurse Menacho at nurse@stmschool.org for assistance.


School Entrance Health Form/Immunization Record

All Pre-K, Kindergarten, and new families must download this form, have it completed by a healthcare professional, and return it to the school nurse.

Permission for Emergency Care
Medication Authorization Form Please complete and deliver with either over-the-counter or prescription medication to the clinic. Forms and medication sent in with students will not be accepted-a parent or guardian must accompany this form.


Severe Allergy Action Plan Download and complete only if your student requires an allergy action plan.


Asthma Action Plan Download and complete only if your student requires an inhaler.


Inhaler Administration Form Download and complete only if your student requires an inhaler.

After-Hours Facilities Use Form The signed consent form must be returned to the STM office no later than one week before you would like to use the facility. Please note that Religious Education and Youth Ministry requests will take priority for classroom space and during the basketball season, CYO Basketball requests will take priority for gym space.
